PAYMENT AGREEMENT

A 40% deposit is due with this application. I understand that the balance is due in full by September 1, 2022. I hereby authorize Westchester Tennis Center to keep my credit card on file to charge the balance to my card if not paid by the due date. I have read and understand the Terms & Conditions and Liability Waiver**

TERMS AND CONDITIONS

By making a payment, the applicant and/or applicant’s guardian accepts the contract enrollment and agrees to the following:

GENERAL:

To hereby remove Westchester Tennis Center(WTC) from any liabilities for injuries while involved in any programs. The use of the facility is strictly at the player’s risk. WTC is not responsible for injuries, accidents or damages to personal property arising from normal athletic activities on the premises. Injuries arising from the interactions with other persons on the court are not the responsibility of WTC. I agree that all participants and their guests hereby specifically waive any claims for damages arising from the use of WTC facilities.

REFUND POLICY:

If any applicant removes him/herself from a WTC program for any reason, or if WTC removes the applicant from WTC program at the sole discretion of WTC Management, including injury of any kind, applicants are still liable for the full cost of the session and are NOT eligible for refunds of any kind.

CANCELLATION & MAKE-UP POLICY:

Private / Semi-Private Coaching

Please email frontdesk@westchestertc.com no less than 24 hours in advance of your scheduled appointment.
Cancellations with less than 24-hour notice are penalized the full lesson fee with no option to reschedule.
Rescheduled appointments or scheduled make-up lessons follow the same 24-cancellation procedure.
You cannot apply make-up lessons towards future private/semi-private enrollments.

GENERAL PHOTOGRAPHY RELEASE:

I hereby grant WTC permission to use my likeness and/or my child’s likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications and social media, without payment or other consideration.
I understand and agree that all photos will become the property of WTC and will not be returned.
I hereby irrevocably authorize WTC to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness and/or my child’s likeness appears. Additionally, I waive any right to royalties or other compensation arising from or related to the use of the photo.
I hereby hold harmless, release, and forever discharge WTC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE.
